SCALING VS. PERIODONTAL FLAP SURGERY: WHEN IS BASIC CLEANING NOT ENOUGH?

One of the most common misconceptions in dentistry is that all gum disease can be managed with routine scaling alone.


While scaling and root planing are highly effective for early-to-moderate periodontal disease, advanced cases often require deeper surgical access to control infection and preserve long-term tooth support.


Clinical signs that may indicate the need for periodontal flap surgery include:


• Deep periodontal pockets

• Persistent bleeding and inflammation

• Radiographic bone loss

• Gum abscess or recurrent swelling

• Tooth mobility

• Heavy subgingival calculus deposits

• Furcation involvement

• Incomplete response after non-surgical therapy


The objective of flap surgery is not simply “surgery.”

The real goal is visibility, accessibility, infection control, and preservation of supporting structures.


When infection extends into areas inaccessible to routine instrumentation, surgical access allows more effective debridement and long-term periodontal stability.


Early diagnosis remains the biggest factor in preventing advanced periodontal destruction.
